Brain MRI Cost in Korea 2026: HIRA Prices and the Headache Rule

발행 한국 비급여 진료비 통계 연구소

Brain MRI cost in Korea depends most on hospital type: HIRA's 2026 medians are 400,000 won at clinics and 768,500 won at tertiary hospitals (1.92x). Since October 2023, simple-headache scans with a normal neurological exam are usually self-paid. Provinces differ less: Chungbuk 400,000 vs. Jeju 620,000 won (1.55x).

목차

A routine brain MRI in Korea costs almost twice as much at a tertiary hospital as at a neighborhood clinic. HIRA's 2026 non-covered price disclosure puts the median at 400,000 won for clinics and 768,500 won for tertiary hospitals (1.92x). The gap between provinces is smaller: medians run from 400,000 won in Chungcheongbuk-do (Chungbuk) to 620,000 won in Jeju (1.55x).

In this report:

How much does a brain MRI cost in Korea in 2026?

In the 2026 non-covered price disclosure, the median routine brain MRI cost 400,000 won at clinics and 768,500 won at tertiary hospitals, a 1.92-fold gap. Dailymedi reported these medians on September 27, 2026. They come from an analysis by Rep. So Byung-hoon of the National Assembly Health and Welfare Committee, based on data from the Health Insurance Review and Assessment Service (HIRA). Each one is a full self-pay price.

Whether a patient pays that price depends on how the scan is billed. A non-covered (bigeubyeo) service falls outside National Health Insurance (NHI) benefits, so the institution sets the price and the patient pays all of it. Covered (geubyeo) services have a national fee, and the patient pays only a co-payment. Selectively covered (seonbyeol geubyeo) services sit in between: insurance applies, but the patient pays a larger share. For brain MRI, that share is 80%.

A Ministry of Health and Welfare (MOHW) notice, "Detailed Standards and Methods for Applying Health Insurance Benefits," decides which patients pay the full price. Since October 1, 2023, it has narrowed NHI coverage of brain MRI for headache and dizziness. The prices are published under Article 45-2 of the Medical Service Act, and Article 45 requires institutions to tell patients their non-covered fees (Korea.kr Policy Briefing; National Law Information Center).

What does the 2026 HIRA disclosure measure for brain MRI?

The disclosure shows what institutions charge for a scan billed as fully non-covered, not what an insured patient pays and not how good the scan is. Health Kyunghyang counts 753 non-covered items in the 2026 round, 60 more than the 693 items in 2025. Of 74,600 eligible institutions, 74,449 submitted prices, a 99.8% submission rate. Since September 3, 2026, anyone can search the results on the HIRA website or in the Geongang e-Eum mobile app. MOHW presents the disclosure as a tool for informed choice. It does not set or cap prices.

A median is the middle value: half of the reporting institutions charge less and half charge more. Rep. So stressed that the gaps appear in the medians themselves, "not merely in outliers." A 368,500-won gap between clinics and tertiary hospitals at the middle of the market is structural. A few expensive providers cannot account for it.

To see where one institution falls in the full range for a sub-item (routine, contrast or MRA), from minimum to maximum, use the HIRA lookup and the brain MRI item statistics page of Korea Non-covered Medical Price Statistics. Institutions report these prices themselves and can change them after the disclosure (Health Kyunghyang).

Which hospitals and provinces in Korea charge the most for a brain MRI?

Tertiary hospitals charge the most, with a 2026 median of 768,500 won. Among provinces, Jeju is highest at 620,000 won.

The median rises with each step up the hospital ladder. The step from general to tertiary hospital adds 218,500 won, which is more than the whole 150,000-won rise from clinic to general hospital.

Two ultrasound items in Dailymedi's report vary more by province. Precision abdominal ultrasound ran from 100,000 won in Jeju to 250,300 won in Sejong (2.50x). Precision breast ultrasound ran from 120,000 won in Chungnam to 259,480 won in Seoul (2.16x).

A cheap province does not guarantee a cheap institution. Jeju had the lowest abdominal ultrasound median, yet its prices ranged from 50,000 to 303,578 won (6.07x). Sejong's prices stayed in a narrow band between 224,000 and 282,000 won (1.26x).

Assuming one routine brain MRI billed as non-covered at the 2026 median, a patient who chooses a clinic over a tertiary hospital pays 368,500 won less, a bigger saving than the 220,000-won gap between Chungbuk and Jeju.

Why does the same brain MRI cost different amounts?

No national fee schedule limits a non-covered price, so each institution sets its own. The Medical Service Act requires institutions to post, explain and report those fees. It does not require them to align their prices.

In Dailymedi's account, Rep. So argued that patients pay these costs themselves, so gaps across regions and institutions call for policy attention. Yoo Ju-heon, MOHW's Director General for Health Insurance Policy, told Health Kyunghyang that the ministry will keep improving the disclosure system after consulting consumers and the medical community. Physicians who criticize the coverage rule argue that its tighter criteria limit the treating doctor's judgment and push borderline cases into self-pay.

The disclosure records only prices. It does not cover scanner specifications, imaging protocols or the quality of the radiologist's reading. The 2026 gaps between hospital types show what hospitals charge. They do not show what patients get for the extra money.

How have brain MRI coverage and prices changed since 2017?

Spending on insured brain MRI grew 12.35-fold after coverage expanded, and the October 2023 rule then moved many headache scans back to self-pay. Medical Times put spending on brain and cerebrovascular MRI at 14.3 billion won in 2017 and 176.6 billion won in 2021. MOHW responded with a February 2023 plan to make health insurance more sustainable. The revised criteria went to the Health Insurance Policy Deliberation Committee on May 30, 2023. They were announced on July 17 and took effect on October 1 (Korea.kr Policy Briefing).

Health Kyunghyang compared 593 items across the market between 2025 and 2026. Average prices rose for 436 items (73.5%), fell for 146 (24.6%) and stayed the same for 11 (1.9%). June 2026 consumer price inflation was 3.2%. Adjusted for it, 448 items (75.5%) fell in real terms, and 145 (593 minus 448, or 24.5%) rose faster than inflation. The gap between institutions widened for 337 items (56.8%) and narrowed for 242 (40.8%). Korea Non-covered Medical Price Statistics calculated both shares against 593.

Price changes for individual items were small. Extracorporeal shockwave therapy rose 1.9% and prolotherapy 0.9%. Dental implants fell 0.9%, and the influenza A/B rapid antigen test did not change.

Three in four prices rose on paper but fell after inflation. Meanwhile, the gaps between institutions grew, so patients who compare prices gain more. For brain MRI, the 2023 rule decides who pays the non-covered price, and the 2026 medians show how much.

Does Korean health insurance cover a brain MRI for headache?

Korean health insurance covers a brain MRI for headache only when the physician's clinical judgment and a recorded neurological exam support it. Since October 1, 2023, every headache or dizziness scan has followed one of three billing paths. The 2026 non-covered medians apply only to the third.

Billing pathConditions (from October 1, 2023)Scan limitWho sets the price
CoveredSuspected brain disease such as hemorrhage or infarction; confirmed brain disease; abnormal neurological exam (for example a cranial nerve or limb motor test)2 combined headache or dizziness scans (previously 3); 3 for thunderclap headache or central vertigo if the reason is recordedNational fee plus co-payment

Sources: Korea.kr Policy Briefing (MOHW, July 17, 2023); Medical Times (2023); Dailymedi (Sept 27, 2026).

Medical Times reports that the old criterion, "persistent headache with two or more of fever, nausea/vomiting or dizziness," was deleted. Cluster headache now requires documented typical features. Examples include severe headache lasting 15 minutes or more, one-sided pain around the eye, conjunctival injection or pupil constriction. Claims must carry a headache or dizziness type code under the JS016 specific-detail code. MOHW plans to focus reviews on institutions that often perform low-necessity MRI. That gives hospitals a reason to bill borderline cases as non-covered.

Two patients show how this works. A patient with long-standing migraine and a normal neurological exam who still wants a scan pays the non-covered price. The benchmark is 400,000 won at a clinic and 768,500 won at a tertiary hospital. A patient with abnormal neurological findings follows the covered path at the national fee.

How do you check a brain MRI price in Korea and review the bill?

Check the HIRA price for your area before you book, confirm the billing path at the clinic, and review the itemized bill after you pay.

  1. Open the non-covered medical fee information service on the HIRA website (www.hira.or.kr) or in the Geongang e-Eum app. Search for the brain MRI item and filter by region, institution type and institution name.
  2. Compare the quote with the 2026 median for that institution type (clinic 400,000, hospital 450,000, general hospital 550,000, tertiary hospital 768,500 won) and for your province.
  3. Ask whether the scan will be covered, billed at the 80% selective benefit or non-covered. Also ask whether contrast or an added MRA is included. Article 45 of the Medical Service Act requires the institution to tell you its non-covered fees.
  4. Confirm the price again on the day of the scan, because institutions can change prices after the September disclosure.
  5. After paying, request the detailed statement of charges (jinryobi sebu sanjeong naeyeok). Check which lines were billed as covered and which as non-covered.
  6. If you believe a covered service was billed as non-covered, file a medical bill review request (jinryobi hwagin yocheong) with HIRA. Any overcharge found is refunded.

What do patients ask most about brain MRI cost in Korea?

What is the difference between covered and non-covered brain MRI?A covered scan has a national fee, and you pay only a co-payment. A non-covered scan is priced by the institution, and you pay all of it. Since October 1, 2023, a simple headache scan with a normal neurological exam is usually non-covered.
What does the 400,000~768,500 won range mean?It spans the 2026 medians from clinics to tertiary hospitals. In each group, half the institutions charge less than the median and half charge more, so individual prices can fall well outside this range.
Why do tertiary hospitals charge 1.92 times more? Each institution sets its own non-covered price, and no national fee schedule applies. The disclosure records the price gap. It does not record scanner specifications or reading quality.
Will insurance cover my headache MRI?Yes, if your physician suspects a brain disease or your neurological exam is abnormal. If you do not fully meet the criteria but brain disease is suspected and the exam is recorded, one scan can be billed at an 80% co-payment. Otherwise you pay the full non-covered price.
How do foreign patients pay for a brain MRI in Korea? It depends on NHI enrollment, which decides whether the covered and 80% paths are open to you. Patients outside NHI should ask how the scan will be billed before it happens. They should compare the quote with the 2026 non-covered median for that institution type.

What should patients check before a brain MRI in Korea, and what changes next?

Institution type has the biggest effect on the price of a routine brain MRI. A tertiary hospital's 2026 median is 1.92 times a clinic's, compared with a 1.55x gap between Chungbuk and Jeju. Under the October 2023 rule, a patient whose headache scan does not meet the criteria pays that price in full. Ask how the scan will be billed before it happens, and request the itemized statement afterward.

These figures apply to a routine brain MRI billed as non-covered in the 2026 disclosure. The numbers change for a contrast scan or added MRA, a covered or 80% billing path, a different province or the 2027 disclosure. Check the HIRA lookup again whenever one of those conditions applies.

Three developments will shape brain MRI prices next:

  • The 2027 annual disclosure will show whether the gap between hospital types narrows or the gaps between institutions keep widening.
  • MOHW's promised improvements to the disclosure system could add fuller price distribution data.
  • Any further revision of the brain MRI coverage criteria would change how many patients pay the full non-covered price.
